In order to provide extraordinary levels of care, we require that patients keep us informed about any changes to their medical history. New patients are asked to complete a confidential medical history questionnaire. As part of this, we will require the name of any medication you are currently taking, as well as any treatment you may be receiving at the hospital. Patients returning to the surgery for routine check-ups are asked to update their medical history as necessary. Additionally, we must be informed about any changes to your address or telephone number. The strict confidentiality of patient records and information is maintained at all times. The records of our patients are not passed on to any third parties without their express and explicit permission.
